For this combo to work, your deck cannot have basic lands. Activate Hermit Druid's ability, which will mill cards from the top of your deck until you reveal a basic land. Since there are no basic lands, you will mill the entire deck. Use Jace's ability and, since you have no cards to draw, you'll win the game.
